The last four digits of your Social Security number (for U.S. players) or your SIN (for Canadians)
A recent utility bill or official document showing your name and address (in some cases)
How to verify:
In the SureShot app, go to the Account tab on lower right, and click Deposit.
Follow the prompts to complete the form.
You should be verified instantly. If there are any typos you will be able to edit and resubmit the form.
